    Since 2017, End of Life Choices Oregon has worked to ensure those facing end-of-life decisions can do so with autonomy, dignity, and compassionate support.

How EOLCOR Supports Your End-of-Life Journey:

Once you contact EOLCOR, you will receive a call from our Services Representative to answer any questions and assess your needs to provide you with the best support. If you qualify for volunteer services and wish to move forward with support, the Services Representative will schedule an intake over the phone with our Intake Coordinator so we can assign a team of volunteers. All information is strictly confidential. 

Once your intake is complete, a client information packet and client forms packet will be emailed or mailed to you depending on your preference. Your volunteers will be in touch to schedule a home visit to go over your end-of-life options and see how they can best support your end-of-life goals and wishes.

We Encourage You to Connect With Our Team As Soon As Possible

If you are interested in utilizing the Death with Dignity Act and meet the eligibility requirements, please contact us as soon as possible. Delaying the qualification process can cause an inability to meet the law’s requirements.

If you are unsure of when or if you want to use the life-ending medication, the pharmacy can hold the prescription until you are ready to use it or not fill it at all. It’s better to qualify as soon as possible so you are able to keep this as an option, rather than wait until your window of opportunity has closed.

EOLCOR has a state-wide network of trained volunteers who can provide expert guidance and support

Our volunteers can:

  • Answer questions and provide detailed information about Oregon’s legal end-of-life options in person, by phone or video.

  • Provide suggestions for talking with your medical team about end-of-life options.

  • Provide referrals, such as hospice and mortuaries.

  • Provide education and assist in organizing care for Voluntarily Stopping Eating and Drinking.

  • Help find qualifying physicians for the Death with Dignity Act and provide support throughout the qualifying process.

  • Provide information and guidance to your healthcare providers about the Death with Dignity Act.

  • Be with you at your planned death if you choose to take life-ending medication.
